If I contribute to my Tradtional IRA between December 31st and the tax filing due date, how does the IRS know I made the contribution since I won't have a tax form?

Go ahead and report the contribution on your taxes then we will send you a 5498 tax form after the Tax Filing Due date showing your contribution that will also be reported to the IRS verifying the information you submitted on your tax return.
